Please note - These forms require a notary which an adult will need to have completed properly. There are people at JMBC that can notarize documents or you can also take the form to the library where there is usually someone that is a notary that can notarize it there, call the library first and ask if a notary is working at that time before going. Remember to not have the form signed before seeing the notary! A current valid copy of your insurance card also needs to be supplied with this form.
